Insurance
All students enrolled in 6 or more credit hours are automatically enrolled in the Limited Student Insurance Plan. The Limited Plan has a per term premium of $26, and no deductible. The Limited Plan is designed to be of benefit to the majority of students. It is a "first dollar" policy, which means it is intended to be billed first when the student already has major medical insurance. It is intended to be of help with co-pays or deductibles of other polices for covered items. There is no prescription coverage under the Limited Plan. .
The SHWC does not bill any insurance. Students receiving care at the SHWC must request an itemized statement and submit the itemized statement with personal information as described below to obtain reimbursement for covered items.
Information about submitting claims
When you seek medical attention, most often, the provider will submit the claims on your behalf to AmeriBen Solutions (the claim administrator). Occasionally, you will need to submit claims yourself.
All you need to do is follow these simple instructions:
- Send all itemized medical and hospital bills (itemized bills will
include provider tax ID#, and diagnosis codes); and all itemized
pharmaceutical receipts (not cash
register receipts), showing prescription number, name of drug, date prescribed, and name of person for whom drug is processed. - Please include the patient's name and insured student's name, address, school
ID#, Somerton medical ID number (from your permanent ID card), and name of
the university under which student is insured with the bills. Claim forms are NOT required. - Send the above information to:
