Directory Information Restriction and Confidentiality

A student's name, local and permanent addresses and telephone numbers, date and place of birth, academic major, enrollment status, participation in officially recognized activities and sports, dates of attendance, degrees and awards received, and the most recent previous educational agency or institution attended is all directory information. According to academic policy, the institution is obligated to release this set of information to anyone who inquires.

A student may opt in writing to keep confidential all "Directory Information" as defined above. The desire to exclude one of the above informational items will result in the exclusion of all items. This option can be made by filing a written, dated, and signed request at the Office of the Dean of Students prior to the close of registration, during the term that the student first enrolls in the current academic year. This option must be renewed each year, remains in effect for the remainder of the current academic year, and the option may not be exercised or changed for the balance of the academic year. The definition was taken from SOU Publication Student Rights and Responsibilities, Section IV, "Administration of Student Records" - (Revised June 1985).

Social Security Number
Disclosure and Consent Statement

You are requested to provide your social security number to assist OUS (and organizations conducting studies for or on behalf of OUS) in developing, validating, or administering student aid programs; improving instruction; internal identification of students; collection of student debts; or comparing student educational experiences with subsequent workforce experiences. When conducting studies, OUS will disclose your social security number only in a manner that does not permit personal identification of you by individuals other than representatives of OUS (or the organizations conducting the study for OUS) and only if the information is destroyed when no longer needed for the purposes for which the study was conducted. By providing your social security number, you are consenting to the uses identified above. This request is made pursuant to ORS 351.070 and 351.085. Provision of your social security number and consent to its use is not required and, if you choose not to do so, you will not be denied any right, benefit, or privilege provided by the law. You may revoke your consent for the use of your social security number at any time by writing to the Enrollment Services Center, Southern Oregon University, 1250 Siskiyou Blvd., Ashland, OR 97520.
